In total, IFCO's San Antonio location racked up 19 violations, 10 of which were classified as "serious." Investigators said the company did not retrain employees on hearing protection, secure dock boards when driven over by forklifts, and they did not have copies of material safety data sheets readily available.

"Serious and repeat violations compromise worker safety," said Jeff Funke, OSHA's area director in San Antonio. "This employer has not exercised its responsibility to ensure workers are not exposed to hazards."

The company also had three repeat violations and six other-than-serious violations, including not logging that an employee experienced a work-related hearing loss.
